Job Summary : We're seeking compassionate and dedicated Personal Support Workers (PSW), Health Care Aides (HCA), or Continuing Care Assistants (CCA) to join our award-winning team.

In this role, you'll provide direct personal support and care to patients, addressing their physical, intellectual, emotional, social, and spiritual needs in accordance with our patient-centered policies and procedures.

Key Responsibilities :

• Provide personalized care to patients, including assistance with ambulation, feeding, bathing, and grooming
• Implement and follow care plans as directed by nursing staff
• Document patient care activities and observations accurately and promptly
• Communicate effectively with patients, families, and other healthcare team members
• Create a safe, comfortable, and dignified environment for patients
• Participate in ongoing training and professional development opportunities

Qualifications and Skills :

• Completion of a PSW Program or HCA / CCA certification that meets appropriate standards
• Experience in Long Term Care or community home care setting (preferred)
• Knowledge of dementia and palliative care (preferred)
• Familiarity with care planning systems and documentation
• Strong communication skills and ability to interact positively with staff, residents / patients, and families
• Proficiency in reading, writing, and understanding verbal and written instructions in English
• Ability to work independently, especially in a Community Home Care setting
• Access to a reliable vehicle for community home care (required)
