Position Status : Full-time(Temporary) End on March 31st, 2025, 37.5 hours per week.

Reports to : ProgramManager

Compensation : $27.92-$29.60

Job Summary :

As a member of the Phoenix Society,Clinical and Outreach Teams based out of the Abbotsford Community Hub, the IntegrationSupport Worker provides a variety of services to those living with viralhepatis and / or HIV currently incarcerated, or recently released into communitywith connections to information about blood borne diseases, communityreferrals, treatment plans, and case planning.

This role includes,facilitation, transportation supports, statistic collection, education,outreach and evaluating service delivery.

Please note that the position istemporary and will end on March 31st, 2025.

Responsibilities : Client Services

Client Services

  • Liaise withother support workers, medical providers and agencies, providing one-on-onesupport to clients who are currently incarcerated and support with transitionalplans to continue their medical treatment once released
  • Outreachinto Federal Institutions, providing discharge planning for inmates who arediagnosed with HCV or at risk of infection
  • Inpartnership with the BCCDC, will deliver educational programming both incorrectional environments and in community
  • Providingaccessible and stigma-free Point-of-Care testing for HCV.
  • Perform newclient intakes as assigned
  • Create andmaintain client case planning documentation
  • Create,include and liaison with the peer support committee
  • DistributeHarm Reduction supplies and resource materials
  • Providetransportation to high-risk clientele for medical care and appointments using personalvehicle, or company vehicle, if available
  • Maintainaccurate case notes and statistics regarding program activities, number ofparticipants, contact and referral information, etc.
  • Maintainconfidentiality and professionalism at all times

Operations

  • Identify areas for improvement and increasedefficiency in program operations, services offered, reporting and reports tosupervisory positions
  • Follow andimplement universal precautions and infection control adhering to all healthand safety protocols
  • Ensure safehandling of sharps containers and other equipment
  • Ensure thatprogram regulations, policies and procedures are followed
  • Maintain up-to-dateknowledge of internal fire safety policies and procedures, as well as other emergency procedures
  • Attendprofessional development trainings as directed
  • Organize andassemble monthly administrative reports
  • Maintainagency standards of work quality

Administration

Provideadministrative support, including but not limited to : scheduling, typing,photocopying, attendance at meetings;

documentation or data entry; creation ofdocuments; or other duties as assigned

Provide reports to the Program Manager asrequested / required

CommunityRelations

  • Supportorganization of community events and attends events providing the public withinformation and awareness resources
  • Facilitationof groups and informational events
  • Participatein liaison activities with stakeholders and the broader community in aprofessional manner
  • Maintain athorough knowledge of social, economic, recreational, and educational resourcesin the community. Liaise with other outreach workers, medical providers, andFraser Health
  • Fosterteamwork within Phoenix Society and with service stakeholders
  • Provide thepublic with information and awareness resources by facilitating groups andparticipating in informational events

Qualifications :

  • Post-secondarydiploma (e.g., 2 to 3 years) in social / human sciences, addictions or other equivalenteducation in the medical field such as LPN or Health Care Aide experience
  • Note : Preference will be given to those whohave post-secondary education and training relevant to the social service andmedical field.

Alternately, those currently pursuing a certification in socialwork, counseling, criminal justice or a related field, or the equivalentcombination of education and experience will be considered for this position.

  • Minimum 2 to3 years’ experience supporting vulnerable populations, involved in the criminaljustice system
  • Minimum 2 to3 years’ experience working with vulnerable populations and communityresources, preferably with clients living with HCV / HIV
  • Understandingof HIV and Viral Hepatitis transmission and treatment is an asset
  • Demonstratedunderstanding of the criminal justice system
  • Satisfactorycompletion of a criminal records check with vulnerable sector screening isrequired; ability to obtain Federal Clearance
  • Satisfactorycompletion of Employee Immunization Record form is required
  • First Aid 1and Current CPR certificate is required
  • Valid driver’slicense accompanied by an approved driver’s abstract is required

  • Knowledge of case management processes and procedures; trained and experienced in professional record keeping, report writing and statistics

Salary for this role is between $27.92-$29.60per hour.
