The AccessAbility Supports Coordinator provides direction and case management support in delivering a range of disability specific supports and services to eligible persons with disabilities.
The AccessAbility Supports Coordinator will manage a caseload that includes strong support for a person centered planning approach related to case management based on needs vs.
entitlement. The Supports Coordinator will collaborate with the program eligible individual, their family and their support network to ensure that the case plan focuses on maximizing the client’s opportunities for full citizenship.
The Supports Coordinator will have significant personal interaction with the client and his / her family and will collaborate extensively with other support agencies, healthcare professionals and community organizations.
The Supports Coordinator provides specialization in the delivery of services utilizing a collaborative case management model with knowledge and expertise on the disability support needs of people with disabilities and working within a multi-disciplinary team.
Duties will include but are not limited to :
- Provide guidance, direction, and support to the individual eligible for the AccessAbility Supports program and their family utilizing a collaborative case management approach;
- Facilitate the development of personalized services and support plans to individuals with disabilities and their families;
- Identify, lead, coordinate and collaborate with the broader community, health and education systems, to ensure the continuity of support plans for clients across the continuum;
- Analyze and interpret results from a complex and comprehensive functional assessment on activities of daily living, that forms the basis of the client support plan;
- Accurately assessing client’s situation, taking into consideration the environmental, individual, familial and social perspective;
- Proactive assessing and planning, as well as being responsive to changing abilities and needs of clients and families;
- Process initial and ongoing applications for services and supports eligible under the program based on needs, and within the defined parameters;
- Assess eligibility for financial supports to address expenses related to basic living assured income for clients, in accordance with program guidelines;
- Respond and assess emergency situations as presented by the applicant / client related to areas such as housing, medical transportation, food, etc.
- deemed necessary to promote independence and self- sufficiency by empowering clients;
- Communicate effectively with clients and / or families, regarding eligibility, support plans, navigation to other service providers or the broader social and health system or community on client’s needs related to their disability;
- Liaison with health professionals and hospital staff for client’s who will be hospitalized for extended periods of time to determine potential in-hospital and transitional supports; and,
- Provide and promote educational and marketing to individuals, public, community, and service organization about supports and services available to persons with disabilities and,
- Other duties as required.
